I'm no expert on this particular app but i do have some experience at apps and cracking etc from the «user» point of view and for what my opinion is worth it sounds like a lot of essential files have been replaced and possibly some deleted with overwriting etc. Usually in this type of situation i would advise to uninstall, remove, delete any installations both full and portable and also search the registry for all instances of «streamfab» and delete any entries for them and simply start from scratch. I say this because:
1. You have already lost settings, log in etc so you have nothing to lose.
2. Even if you do get it working there is no guarantee it will stay working, especially with any future updates for that particular version. Things not only work best from a clean install but are far less likely to go wrong anless anything major happens to the installed app or hard drive.

I have 6.1.6.5 working but still not working on channel 4 or channel 5 (my5) both are UK channels. They are the only 2 channels i really need to work.
